At D R Downes & Associates, we provide experienced psychological care for individuals navigating complex personal and professional environments.
All clinical work is delivered by senior psychological practitioners who have been practising since 2008. Over the past fifteen years we have worked alongside professionals operating in demanding roles across sectors such as finance, engineering, aviation, healthcare, and business leadership, where sustained responsibility and complex decision-making are part of everyday life.
Based in Canary Wharf, London, the practice supports clients across the UK and internationally through secure remote sessions. We regularly work with professionals living and working in global business centres, including the UAE and wider Middle East, and understand the pressures that come with operating across different systems, cultures, and time zones.

Our practice is grounded in many years of professional experience working with individuals in complex and demanding environments. This experience allows us to remain steady, thoughtful, and clear when clients are navigating pressure, uncertainty, or significant life challenges, providing a calm and confidential space where difficult issues can be understood and worked through.

Our approach is grounded in a biopsychosocial understanding of health and informed by the developing NSCF–CTA™ framework, which explores how capacity, perceived threat, and agency interact within the nervous system under sustained load. Rather than adhering to a single model, we draw thoughtfully from approaches such as EMDR therapy, psychotherapy, and cognitive behavioural therapy (CBT), adapting the work to the individual and their circumstances.

Born in Dublin and now based in the UK, David brings a background spanning operational service, senior corporate leadership, and psychological practice. He served in the Royal Navy Submarine Service before moving into executive roles in the private sector, including Managing Director of a public company, and also holds a Private Pilot Licence.
Practising since 2008, David provides senior psychological support to individuals navigating demanding personal and professional environments. With experience drawn from more than 15,000 one-to-one sessions, his work is valued for its clarity, pragmatism, and steady perspective when people feel mentally stuck or under sustained pressure.
David also provides retained psychological support to corporate organisations, offering senior-level consultation where leadership teams and professionals are operating in complex or high-pressure environments.

Rebecca Downes brings a background spanning strategic HR, organisational development, and psychological practice. Prior to becoming a psychotherapist, she worked extensively within organisational environments, supporting leaders, teams, and professionals navigating high-pressure roles, complex change, and sustained responsibility.
This experience continues to shape her clinical perspective. Rebecca understands the intersection between personal wellbeing and organisational demands, particularly for individuals operating in complex professional systems.
Alongside private clinical practice, Rebecca also provides psychological consultation and retained support within organisational settings and is the developer of the emerging NSCF–CTA™ framework, which explores how capacity, perceived threat, and agency interact within the nervous system under sustained load.
Our clinical work integrates established psychological approaches including EMDR therapy, psychotherapy, cognitive behavioural therapy (CBT), somatic -informed CBT and resilience-focused coaching. These approaches are delivered within a biopsychosocial understanding of health, recognising that psychological wellbeing is shaped by the interaction of biological processes, psychological experience, and the social and professional environments in which individuals live and work.
This work is further informed by the developing NSCF–CTA™ framework, a clinical architecture created by Rebecca Downes. The framework integrates with the biopsychosocial model by offering a mechanism-focused lens for understanding how capacity, perceived threat, and agency interact within the nervous system when individuals are under sustained load.
From this perspective, many psychological and functional symptoms can be understood as protective responses that emerge when the system’s adaptive flexibility narrows under pressure. Clinical work therefore focuses on restoring flexibility within the system — helping individuals rebuild capacity, recalibrate threat responses, and regain a stable sense of agency.
The aim is to support movement beyond symptom management toward a more sustainable and adaptive way of functioning.
Experience is central to this work. It allows practitioners to remain steady, thoughtful, and clear when individuals are navigating demanding circumstances, offering a confidential and carefully held space in which complex experiences can be explored and understood.

Our work is neurodiversity-aware, informed by specialist training and lived understanding of difference.
We adapt how we work to different ways of thinking, processing, and regulating, recognising that neurodivergence shapes experience in varied and individual ways. For some clients this is central to the work; for others it simply forms part of the background to thoughtful, individualised psychological care.

Alongside psychotherapy, we integrate resilience-focused psychological support where it is helpful.
This may include supporting regulation under sustained pressure, managing stress and emotional load, working with the impact of demanding roles or critical events, and strengthening the capacity to think clearly and respond rather than react.
Resilience work is always grounded in psychological understanding and tailored to the individual never delivered as a generic programme or bolted on as an afterthought.
We work primarily remotely, supporting clients wherever they are. Remote work offers privacy, flexibility, and continuity, particularly for those living or working internationally, relocating, or managing demanding schedules.
A very limited number of in-person sessions are available in London and Chester, by arrangement, where clinically appropriate and availability allows.
We also accept a small number of insurance-funded cases, which are intentionally limited to protect clinical quality, continuity, and focus.
D R Downes & Associates operates as a sole-trader practice, supported by a husband-and-wife professional working relationship.
Each practitioner works independently with their own clients, bringing complementary experience within a shared professional ethos. This structure allows for depth, consistency, and discretion, without the impersonality of larger organisations.
The practice is small by design, not by limitation.
Our work typically supports individuals operating in high-responsibility roles, including senior professionals, executives, and business leaders. Many are outwardly functioning well but recognise that sustained pressure, complex decisions, and demanding environments can begin to affect clarity, resilience, or wellbeing.
Clients are often self-funding and value discretion, flexibility, and the opportunity to work directly with experienced practitioners in a confidential space where complex challenges can be thought through clearly.
